In small blue tupperware in the second little drawer below and left of the phone. There are cut up blank forms with the change. Some that have already been filled out by some of us. Just cut up a few more sheets as you need them and make more from the master (the last one) when those run out.
Sharon picks up the forms approx. once a month and replenish the change. Call her if we're running low on $.
